To view forms and agreements of the programs you have registered to, follow the steps below.
Step 1: Log into your athlete one account Click Here
Step 2: Select 'Programs/Registration’ from the left hand navigation, then select your child from the fly-out window.
Step 3: Select the program your looking to add Forms & Agreements to under 'Current Programs'.
Step 4: Select 'Forms & Agreements'.
Step 5: Here you can view or upload any required forms.
