To view forms and agreements of the programs you have registered to, follow the steps below.

Step 1: Log into your athlete one account Click Here

Step 2: Select 'My Account' from the left hand navigation > then select 'My Children’

Step 3: Select your child from the fly-out window then select 'Programs' on the left hand side.

Step 4: Select the program your looking to add Forms & Agreements to under 'Current Programs'

Step 5: Click 'Forms & Agreements'

Step 6: Here you can view or upload any required forms.
