To view forms and agreements of the programs you have registered to, follow the steps below.
Step 1: Log into your athlete one parent account - Click Here
Step 2: Select 'My Accounts' from the left hand navigation > 'My Children'.
Step 3: In the fly-out window, select your player, and in the drop-down, click 'Programs/Registration'.
Step 4: Select the program your looking to add Forms & Agreements to under Current Programs.
Step 5: Select 'Forms & Agreements'.
Step 6: Here you can view or upload any required forms.
